Any place to get an entry form or rules online? If we made it up we wouldn't be in the area till friday night. Thanks

Please note, the entry fee for the derby is $20, not $25 as I previously posted. The Ludington Grill will be open until at least 10PM on Friday. Entries must be in by Friday night. No online entry forms are available. If you can't sign up by Friday night, try calling them at (906) 553-7778. They MIGHT let you sign up over the phone and pay by credit card, (can't hurt to ask.)
GENERAL RULES-
Cash prizes based on weight in walleye pike, and perch categories. Length will be used in the event of a tie. 100% payback.
Fishing starts at 5AM on Saturday, February 20th. Fishing boundaries are from the head of the bay to Portage Point.
All fish must be registered at Ludington Grill (back patio,) by 8PM sharp! Use the back entrance - don't bring fish through the restaurant!
Follow all Michigan fishing rules and regulations. Hook and line only - no speared fish.
Must be 18 to participate, unless accompanied by an adult.
All fishermen must fill out and sign waiver of liability form. Your signed waiver form must be on file in order for eligibility.
Officials will be present on the ice.
One win per person. If you win in any one category, you may not win in another.
Only "fresh" fish will be weighed. No frozen fish may be entered.

1st place - Scott Petereson, 10 pounds, 12 ounces
2nd place - Chad Carlson, 9 pounds, 8 ounces
3rd place - Brian Houle, 9 pounds 2 ounces
PIKE
1st place - Luke Maulding, 6 pounds 3 ounces
2nd place - Erik Sexton, 6 pounds
3rd place - Rick Hakes, 4 pounds 4 ounces
PERCH
1st place - Mark Morgan, 1 pound 1 ounce
2nd place - Pat Grenier, 15 ounces
